
The Mount Forest Family Health Team is funded by the government, through the Ministry of Health. We are working closely with all of the doctors in Mount Forest, but for convenience, most of our services are housed in the Claire Stewart Medical Clinic across from the hospital.

How Do We Work as a Team?
Each of the different types of health care providers (e.g. doctor, nurse practitioner, nurse, social worker, pharmacist) has a different set of skills and range of knowledge to bring to helping you to improve your health.
These people work together either behind the scenes or during your visit, to make sure that all of the issues are looked at and all options are considered when making a plan for your health care.
You play a key role in this planning. Please ask questions and offer suggestions.
What Are We Trying to Achieve?
No one health care provider alone can help you with all of your health care needs. Our goal is to provide a wider range of health care services to people who live in:
- Wellington North
- Minto
- Southgate
- West Grey; and
- Surrounding areas
How to Make an Appointment
Our services are free to anyone who wishes to receive them at our service location. They are paid for through your taxes. Our health care providers work with all ages, groups, backgrounds and income levels.
You may refer yourself or have your doctor make a referral.

SERVICES
NOTE: Although we are currently funded for the services listed here, not all positions are filled at this time.
Social Work
Social workers help people resolve problems that affect their day-to-day lives.
People see social workers when they are going through a difficult period in their personal, family and/or work life.
Social workers help clients:
- identify and understand the source of stress or difficulty
- develop coping skills and find effective solutions to their problems
- find needed resources
by providing counselling and psychotherapy.
Clinical Pharmacist
Clinical Pharmacists help people to:
- understand the medications they are taking
- make decisions about medication choices
- understand how food, activity and other medications affect the medication they are taking.
The pharmacist also works with the other members of the team to provide information about drug interactions, medication choices and the best way to help you if medication is an option.
Nurse Practitioner
Nurse Practitioners are specially trained and registered nurses. Their skills include the ability to:
- provide wellness care including health screening activities such as Pap smears and monitoring infant growth and development
- diagnose and treat minor illnesses such as ear and bladder infections
- diagnose and treat minor injuries such as sprains and lacerations
- screen for the presence of chronic disease, such as diabetes
- monitor people with stable chronic disease, such as hypertension
Nurse Clinicians
Nurse clinicians are registered nurses who work with the rest of the team to help you to improve your health. Nurse clinicians are able to:
- answer questions about how you can feel better and improve your health
- provide information on different health topics either one on one or in groups
- provide support for any changes you are making to improve your health
The nurse clinicians may also provide health screening services through individual appointments or clinics such as blood pressure monitoring.
